How do I obtain an accident report? If you have been involved in a motor vehicle accident in the City of Waseca, reports are available through the Waseca Police Department. State Accident Reports and/or Calls For Service concerning the accident are available for a fee of $5.00 for the first page, and $1.00 for each additional page, payable by check, cash or money order at the Police Department window. Photos can also be printed for $2.00 per page. Call (507) 835-9720 to inquire whether your report is ready and available, or stop at City Hall. Reports are only available to the person involved in the accident or the parent of a juvenile. If an insurance company needs a copy of a report, the person insured must sign a release with the company so the Police Department can send or fax the information. The Police Department cannot fax an accident report without a signed release as proof of the person involved requesting it. Persons involved in an accident involving $1,000 or more in property damage, or injury or death, MUST COMPLETE this form and send it to Driver and Vehicle Services within 10 days. Blank forms are available at the Waseca Police Department as well. Failure to provide this information is a misdemeanor under Minnesota Statute 169.09, subdivision 7.
